I'm inferring from your question that the coverage is never to have been effective -- all the way back to the beginning of time. In this instance, CoverageTerminationDate would typically be one day before the CoverageEffectiveDate. In other words, the coverage is effective for 0 days.
The coverage might also have an event associated with it that has a EventReasonTypeCode of "Cancellation", but that is not a given. This situation could also arise during open enrollment or as a result of some valid qualifying event.
The standard currently lacks a ReasonTypeCode for withdrawal.
